Welcome. Here's how your first visit works.
Most of the paperwork can be done from home before you arrive. Below you'll find the forms, what to bring, how billing works and the policies we ask every patient to know.
Three steps, about twenty minutes
-
01
Request an appointment
Book online or call. We'll ask for your insurance details so we can verify coverage before you come in.
-
02
Complete your forms
You'll get a portal invitation by email. Fill in your history and consents there, or print the PDFs below and bring them along.
-
03
Arrive 15 minutes early
Check in at the front desk with your ID and insurance card. Your first visit is longer than usual so your provider can get the full picture.

Insurance & billing
We accept most major plans, including Medicare and [STATE MEDICAID PROGRAM]. Call the front desk to confirm yours before your visit, since plans change often.
- Copays are due at check-in. We take cards, cash and HSA/FSA cards.
- No insurance? Ask about our self-pay rates; a good-faith estimate is available on request.
- Statements arrive by mail or in the portal. Pay online, by phone or in person.
- Billing questions: [BILLING PHONE], Mon–Fri 9am–4pm.
The few rules that keep things running
Please give 24 hours' notice so we can offer the time to someone else. Repeated no-shows may incur a [FEE] charge.
If you arrive more than 15 minutes late we may need to shorten or reschedule your visit to stay on time for others.
Request refills through the portal or your pharmacy. Allow three business days. Controlled substances require a visit.
Results post to your portal as soon as they're reviewed, usually within two business days. We'll call about anything urgent.
Call [PHONE NUMBER] and follow the prompts to reach the on-call provider for urgent concerns that can't wait until morning.
Patients under 18 must be accompanied by a parent or guardian, or bring a signed consent form for the visit.
Still wondering about something?
If your question isn't here, the front desk is happy to help at [PHONE NUMBER], or use the contact form.
Plan for 45 to 60 minutes. Your provider will review your history, do an exam and set up a plan for anything ongoing. Follow-up visits are usually 20 minutes.
Yes. Browse our providers and request anyone accepting new patients. If you'd like the soonest opening, choose "first available" and we'll match you.
Sign the release of records form above and we'll request them for you. It can take a few weeks, so the earlier the better. Bringing copies to your first visit also works.
Free patient parking is in the lot behind the building, with accessible spaces by the entrance. Details and transit options are on the contact page.
Of course. You're welcome to bring one support person. If you'd like us to share information with them when you're not present, complete the authorization section of the registration form.
Ready when you are
New patient visits are usually available within two weeks. Request a time and we'll take care of the rest.
