Terms of Use
These terms govern your use of the Harbor Family Medicine website. By using the site you agree to them. They do not replace the consent forms and policies you sign as a patient.

The website is not medical advice
Content on this site is general health information for educational purposes. It is not a substitute for a consultation with a qualified clinician, and reading it does not create a provider–patient relationship. Decisions about your health should be made with your own provider, who knows your history.
Emergencies
Do not use this website, its forms or the patient portal for urgent or emergency needs. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Messages sent through the site are read during office hours only.
Appointment requests and online forms
- Submitting an appointment request does not confirm a time. An appointment exists only when our office confirms it by phone, text or email.
- You agree that the information you submit is accurate and that you are the patient, a parent or guardian, or otherwise authorized to act for the patient.
- The general contact form is not a secure channel for health information. Please keep health details for the patient portal or a phone call.
- By providing a phone number or email you agree that we may use it to respond to your request and send appointment-related messages. Message and data rates may apply. Reply STOP to text messages to opt out.
Patient portal
The patient portal is a separate service provided by [PORTAL VENDOR] under agreement with us and has its own terms, which you accept when you create an account. You are responsible for keeping your login credentials private and for all activity under your account. Tell us right away if you believe your account has been accessed without permission.
Acceptable use
You agree not to use the site to do anything unlawful, to interfere with its operation or security, to attempt to access information that is not yours, to submit false information, or to send unsolicited commercial messages. We may block access to anyone who violates these terms.
Intellectual property
The text, graphics, logos and layout of this site belong to Harbor Family Medicine or are used with permission. You may view and print pages for personal, non-commercial use. Any other reproduction or distribution requires our written consent.
Third-party links
We link to outside websites, such as insurance carriers, health organizations and mapping services, for your convenience. We do not control those sites, are not responsible for their content or privacy practices, and a link is not an endorsement.
Disclaimers and limitation of liability
The site is provided "as is" and "as available." While we work to keep information accurate and the site running, we make no warranties, express or implied, about its completeness, accuracy, availability or fitness for a particular purpose. To the fullest extent permitted by law, Harbor Family Medicine and its providers and staff are not liable for any indirect, incidental or consequential damages arising from your use of, or inability to use, the site. Nothing in these terms limits the professional duties we owe you as a patient, which are governed by applicable law and your care agreements.
Governing law and changes to these terms
These terms are governed by the laws of the State of [STATE], without regard to conflict-of-law principles. We may update these terms from time to time by posting a revised version here with a new "last updated" date. Continued use of the site after a change means you accept the revised terms.
